# Patient-access completion record checklist

Use this checklist to inspect an administrative patient-access workflow. It is
educational material, not clinical or legal advice and not evidence of a live
Koltra product run.

- [ ] The request has a channel, time, stated administrative need, and permitted identity state.
- [ ] Required, present, missing, and ambiguous facts are distinguishable.
- [ ] The exact scheduling rule, location, appointment type, and approval boundary are recorded.
- [ ] Any claimed booking or change has an appointment-system result and safe reference.
- [ ] Symptoms, clinical questions, triage, care advice, and unsupported exceptions stop for an authorized person.
- [ ] Every handoff records its reason, attached context, accepted owner, and acceptance time.
- [ ] The caller-facing confirmation agrees with the appointment-system or accepted-owner state.
- [ ] The workflow ends as completed, human owned, blocked safe, failed contained, or unresolved.

Never report completion without an authoritative system result or an accepted
human owner.
